What is the Gastric Band?

The Gastric Band is a silicone ring that limits the entry of food into the stomach, working like an hourglass. It produces a quick sensation of fullness and facilitates the patient's compliance with the dietary program. Prevents stomach dilation below the band.
It is currently used as part of 'revision surgery' (failed surgery or insufficient weight loss) and 'as a booster for primary obesity surgery'. The use of gastric banding as a unique and isolated technique is currently in disuse. Know more details at the bottom of the page.

In which cases the Lap Band is used:

As part of a "Revision Surgery", when a patient who has already undergone surgery for obesity but whose operation has failed is surgically reviewed. to reinforce or improve the effectiveness of an obesity operation that has partially failed.
As "part of a primary surgery of obesity", when it is foreseen that it is necessary to give more strength to an operation of obesity and to avoid that the patient dilates the stomach in a future (the Gastric Band plays a complementary role to another technique, Gastric Sleeve or Gastric Bypass or more frequent).

Previously the Lap Band has been a technique widely used by itself, for the treatment of obesity. There are two types of Gastric Band: 1- Adjustable Gastric Band, which can be regulated through a small valve that is implanted under the skin in the abdomen and allows to open or close the passage of food in a very simple way. 2- Fixed band, not adjustable, adjusts only the time it is implanted.

Adjustable Gastric Band:

- It is a kind of ring that is implanted in the upper part of the stomach to limit the entry of food into it. Contains a chamber inside, which when swollen, decreases the size of the food passage hole, increasing its restrictive effect.
- The swelling is done through a "port" or valve system that is implanted under the skin of the abdomen. The swelling is simple in expert hands, without the need for anesthesia, ambulatory and in a few minutes. Radiological monitoring may be necessary.

It is a system composed of several parts, which can fail over the years, although less than 10-15% of the cases occur.
If you swell excessively in search of greater restriction, you may injure your stomach.
If the patient forces and tries to eat more than the Lap Band allows, the patient vomits and the band can be mobilized, and in that case injure the stomach. In the case of mobilization, it must be completely disintegrated and removed by surgery.

Fixed Gastric Band:

This is a ring of non-absorbable material, which is implanted in the upper part of the stomach after the obesity operation (most often the Gastric Tube / Sleeve or the Gastric Bypass). Everything is done in the same surgical procedure. Polypropylene or goretex bands are used, 1cm wide by 7 cm long, of which 1cm is used to fix it on itself and on the stomach. There are "rings" marketed, which have not proved more effective and have a higher economic cost.

It is exceptional that it can damage the stomach and force an operation, except for excessive restriction.

The adjustment is made only once at the time of surgery.
It doesn't allow you to adjust without surgery.
Some patients may have excessive restriction with frequent vomiting and impaired quality of life. Food re-education is a good solution in these cases.
It may goretex, less frequently produces excessive restriction and punctual vomiting, but may expand minimally over time, and be less effective.

Removal of the Adjustable Gastric Band:

Sliding or mobilized band, which has forced the complete deflation of the same.
A band that is injuring the stomach wall (migration is the most common name).
- Migration may be asymptomatic initially.
- The removal of the band should be done as a priority by an expert team, as soon as possible.
- One of the symptoms of migration is the apparent infection or inflammation of the swollen port area in the abdomen.

This must be done by an expert team in order to avoid major injuries.
It is performed by laparoscopy, with hospital discharge on the same day.
You can make the removal and in the same surgical procedure, another surgical technique of obesity (Tube - Gastric Sleeve or Gastric Bypass)
In case of migration - injury to the stomach wall - a double approach is most often necessary, bariatric surgeon by laparoscopy and digestologist by endoscopy. In these cases the income and recovery is longer.
