What is the Gastric Sleeve?

The operation of Sleeve or Gastric Sleeve (also called Gastric Tube), is one of the most performed techniques and has the advantage of not requiring long-term supplementation with a proper diet.
The aim is to reduce the size of the stomach, removing the major curvature that allows it to dilate and have an increased capacity. In this way, the noble parts of the stomach remain intact and are fully functional.

How does the Gastric Sleeve work?

By modifying the structure of the stomach is achieved:

Decrease in gastric capacity: A dessert plate in the middle of a flat plate.

Patients with BMI 35-45 Kg / m2:

Patients with a BMI> 60 (super obesity):

Gastric bypass would be the ideal technique in this case, however, these users involve great technical complexity and a more deteriorated state of health.
As the 1st phase, the Gastric Sleeve can be carried out, with which the patient usually reaches a BMI lower than 35.
The Gastric Sleeve goes to Gastric Bypass in case the patient has not lost enough weight or if he begins to regain the lost weight, preventing the BMI from being much higher, thus reducing the risk of the operation.

The weight loss produced is variable and may be due to:

Gastric sleeve surgery is one of the most used techniques to combat obesity in the world. It is a restrictive surgery, it reduces the capacity of the stomach, and it is definitive. Through mini-invasive or robotic laparoscopic surgery, the stomach is removed, the “pocket” that allows us to dilate the stomach during the meal and significantly increase its capacity. However, the important parts for its function are left undamaged: the inlet valve, the outlet valve, the gastric body and antrum (reduced), the vagus nerves and gastric arteries. In this way, the stomach works normally and you can eat all kinds of food. What is limited is the capacity.

The duration of gastric sleeve surgery varies greatly according to each patient and the experience of the team. At Clínica Obésitas, with an experience of more than 550 patients operated on with this technique, we performed the operation by mini-invasive laparoscopy, and with a duration of about 60 minutes, plus the anesthetic induction and awakening time. With our safe and minimally invasive methodology, patients walk around their room, drink fluids in a few hours, and are discharged from the hospital the next day. In addition, they are allowed to walk 1 hour a day at discharge, because the state of comfort and health is very favorable and this prevents possible clinical complications (not directly related to surgery) typical of obese patients after an operation.

The risks of gastric sleeve are mainly the following:

1- Possible complications in the immediate postoperative period:

Deep vein thrombosis: 0.5%, pulmonary embolism, non-fatal thrombus: 0.5%, stomach suture leak and fistula: <1%, postoperative bleeding <1%. Mortality rate: 0.25% (These figures are still lower in teams and centers of excellence. At Clínica Obésitas, with an experience of more than 1400 patients treated in 17 years, we have a mortality of 0%, a leak rate of 0.07% (1 case).

2- Possible complications in the postoperative period in the medium long term:

Gastric dysfunction, Gastroesophageal Reflux, Gastric stenosis. Possible dilation over the years. Most of these problems are rare and can always be resolved, in expert hands, with medical or surgical treatment. Some complications, including weight gain, may force over the years to change the operation, transforming the Gastric Sleeve into a Gastric Bypass or a SADIS Bypass.

The benefits of gastric sleeve are progressive and healthy weight loss, without the need for long-term vitamin supplementation. This technique leaves a smaller stomach, but it works normally. Food absorption is not affected. The small intestine is unchanged. The result is that the patient feels satiety and satisfaction with a lower intake, but at the same time can be nourished without any impairment to health, and long-term vitamin supplementation is not necessary (yes initially during rapid, initial weight loss ).

Weight loss after gastric sleeve is variable, depending on the initial obesity, age of the patient …, etc. Normally a BMI of less than 30 is reached, weighing about 10 kilos more than the last two figures of the height. If I measure 1.60 mt. I stay at about 72-75 kg. Patients under 35 years of age easily reach normal weight (60 kg for the previous example). The quality of the surgical technique, as well as the support and training of the patients during the months following the operation, influences all of this. You can eat after a gastric sleeve, all kinds of food. Although you must maintain a balanced and healthy diet. Initially, just after gastric sleeve surgery, the patient must feed with progressive textures. During the first month after the operation, with a liquid, semi-liquid, soft and normal phase. From here, the first months the patient may have certain difficulties with dry food (meat, breast, paella-type dry rice) and feel more comfortable with spoon dishes, also with stewed meat, etc. Over the months, patients tolerate all foods most of the time.

The difference between the gastric sleeve and the bypass is the intestinal bypass that is performed in the bypass. The gastric sleeve is a restrictive technique only (the size of the stomach is reduced) while the gastric bypass is a more malabsorptive restrictive technique (the stomach is reduced and the first part of the duodenum and ileum -small intestine- is derived). The effect on the patient in the gastric sleeve is a decrease in the food that can fit in the stomach, associated with a decrease in ghrelin and the craving to eat. In gastric bypass, in addition, a decrease in the food that can fit in the stomach, the food does not pass through the first meter and a half of the intestine, and with this a decrease in the absorption capacity of fats and an improvement in balance is achieved. of the absorption of carbohydrates (improves diabetes and prediabetes typical of obesity) However, gastric magic is indicated mainly in patients with less significant obesity (BMI <45), and younger people and women with possible pregnancies futures, although also in those older than 70 years, due to its simplicity.
